The most underused tool in youth sports

If you could give a girls' soccer, basketball, or volleyball team one intervention that meaningfully reduces their risk of ACL and other non-contact injuries, costs almost nothing, and takes less time than a typical dynamic warm-up, would you use it?

That intervention already exists. It's called neuromuscular training (NMT), and research consistently shows that structured NMT programs — such as the FIFA 11+ or the PEP (Prevent injury, Enhance Performance) program — reduce non-contact lower-extremity injury rates in adolescent female athletes when performed consistently, two to three times per week, in-season.

What "neuromuscular training" actually means

Neuromuscular training isn't a single exercise — it's a category of training designed to improve the communication between an athlete's brain, nerves, and muscles so that her body reacts to sport situations (cutting, landing, decelerating) with better joint alignment and less reliance on ligaments to absorb force.

A well-built NMT program typically layers together:

  • Dynamic warm-up movements that raise tissue temperature and prime movement patterns
  • Strength exercises targeting the hip, glute, and core stabilizers
  • Plyometric drills that teach safe landing mechanics and force absorption
  • Balance and proprioception work on single-leg and unstable surfaces
  • Agility and cutting drills performed with cueing on knee position and posture

What the evidence says

Foundational work by Hewett and colleagues, along with the widely cited FIFA 11+ and PEP program trials, found that structured neuromuscular training programs are associated with meaningfully lower rates of ACL and other non-contact knee injuries in adolescent and young adult female athletes. These programs share a common structure: a brief, consistent, whole-team warm-up replacement performed multiple times per week.

Why girls benefit disproportionately

Neuromuscular training helps all athletes, but the evidence base specifically targeting female athletes exists because of well-documented, biology- and biomechanics-linked differences that emerge around puberty — including changes in landing strategy, quadriceps-dominant movement patterns, and relative hip and core strength. Building a 12-15 minute NMT block

You do not need special equipment, a strength coach, or extra practice time. Most effective programs replace the traditional "jog and stretch" warm-up entirely. A simple structure:

Practice tip

Consistency beats complexity. A team that does a simple 12-minute NMT circuit three times a week for a full season will outperform a team that does an elaborate, coach-intensive version sporadically. Pick a version you will actually repeat.

Coaching cues that matter most

The exercises only work if athletes perform them with attention to quality, not just repetition count. When coaching landings and cuts, prioritize these cues:

  1. "Soft knees, not locked knees" on landing
  2. "Knees over toes" — avoid inward collapse
  3. "Land quietly" — a loud landing usually signals poor force absorption
  4. "Chest up, hips back" to encourage hip-dominant deceleration
  5. Stick the landing and hold for two full seconds before moving again

Common implementation mistakes

Even well-intentioned programs fail when:

  • They're treated as optional. NMT works because of dosage and consistency — occasional use produces little measurable benefit.
  • Form is not corrected. Athletes performing plyometrics with poor mechanics may not get the intended benefit; a coach or trained volunteer should watch and cue.
  • It's cut when time is tight. Ironically, the days practice feels rushed are exactly when a fatigued team most needs the deceleration and landing reminders NMT provides.
  • It stops after preseason. In-season maintenance matters — the evidence base for programs like the 11+ assumes year-round or full-season use, not a two-week preseason block.

Caution

Neuromuscular training is a powerful risk-reduction tool, but it does not make injury impossible. It should be paired with adequate rest, sport-appropriate load management, and — where a prior injury exists — a full, criteria-based return-to-play process rather than a fixed timeline.
